Notes for applicants 
Before completing this application form, please ensure that you meet the relevant eligibility criteria published on our website. All applications should include a named referee who can confirm to us that they are aware of your care giving responsibilities. They will be contacted as part of the application process and must be your Somerset Council Family Intervention Worker, Young Carer Lead Worker, Social Worker or it could also be a teacher at your school who knows you well and recognises you as a Young Carer.

Why you need a grant
We understand that as a Young Carer it can be difficult to spend time as a family on outings or short breaks. You can apply to this fund for a grant to help pay for an activity, day out or short break that you would like to enjoy, either with your family, on your own, or with a mentor or friend.

You can use this grant for the following things:
  • buying tickets for visiting an attraction or event (this could include an annual pass for example)
  • transport costs to the attraction or event you would like to go to
  • accommodation costs for a short break
  • support costs that may be needed for the person in your family who you care for to enable them to be with you on the trip or for them to be supported whilst you are away.
  • food costs whilst on your day out
The maximum grant size is £500. If the total cost of what you want to do is more than £500 you can use the grant as a contribution to the overall cost as long as you can confirm to us that you have the remainder of the money available.







Your Referee's Information

Your referee needs to be someone who knows you well and who can comment on why you are suitable for the grant you have requested. 


They will be contacted as part of the application process and must be your Somerset Council Family Intervention Worker, Young Carer Lead Worker, Social Worker or it could also be a teacher at your school and who recognises you as a young carer

  • I am applying for a grant from the Somerset Community Foundation and that the information I have given in this form is correct. If any of the information is found to be knowingly incorrect, I may be ineligible for future grants.
  • If my circumstances change and this is relevant to my application I will contact Somerset Community Foundation to let them know. 
  • I will be contacted by email regarding this application and any grant awarded and so I will regularly check my emails and junk mail folder. This will include confirming receipt of any grant payment and providing feedback to Somerset Community Foundation. 
  • I am aware that if a grant is awarded I will be asked to provide feedback on how the grant was spent and how it benefitted me within 6 months of receiving the grant payment. 
  • Somerset Community Foundation may share anonymised information provided in this application with our relevant funders.
